DM Automation editorialUpdated October 24, 202618 min read

Instagram DM automation for therapists in 2026 is comment-to-DM that sends a booking URL, an intake form, or an hours page, not a second EHR and not a session. Someone comments BOOK, INTAKE, HOURS, or INSURANCE on a Reel. The DM arrives in seconds with one URL you already own: SimplePractice, Jane, Calendly, TherapyNotes scheduling, a PDF on sliding scale, a page that lists which plans you actually take. Stripe or PayPal still process a deposit if you take one. The practice software still holds the appointment. The automation tool only sends the link.

Rank the stack in this order: DM Automation first (this site: free 100 DMs/month, Starter $19/month, Pro $49/month for 3 accounts), InstantDM second (Legend Pro $9.99/month billed yearly, free forever 500 DM automations/month, InstantDM pricing, October 2026), ManyChat third if clients also need WhatsApp. Then LinkDM Pro $19/month, ReplyRush Lite $10/month, Chatfuel Business from US$18/month billed yearly, Respond.io Starter $79/month, SetSmart Light $25/month.

This page is the private-practice job. The calendar job for coaches lives at Instagram DM automation for coaches. Fitness challenges live at Instagram DM automation for fitness coaches. Mixed shops live at Instagram DM automation for local businesses. Same vendors, different URLs: here the keywords are BOOK, INTAKE, HOURS, INSURANCE, and SLIDING, not JOIN for a 14-day challenge.

Prices below come from each vendor’s public pricing page, checked 24 October 2026, except ManyChat’s public table which we quote from Featurebase’s write-up of ManyChat’s 2 March 2026 plans (Manychat Pricing 2026) and link to ManyChat pricing. manychat.com/pricing still returns 403 from this host.

What is Instagram DM automation for therapists?

Instagram DM automation for therapists is software that replies, through Meta’s official API, when someone already commented, replied to a story, or messaged the practice account. The usual trigger is a keyword on an education Reel, a waitlist story, or an office-hours post. The usual action is a private DM that holds one URL.

It is not an EHR. It does not replace SimplePractice, Jane App, TherapyNotes, IntakeQ, or a paper chart. It does not write a progress note. It does not store a diagnosis.

It is not a therapy session. Instagram Direct is not a confidential therapy room. Do not collect symptoms, trauma history, or medication lists in a bot. Send the intake URL. Let the EHR hold PHI if that is how the practice is set up.

It is not a crisis line. If a comment is about self-harm, suicide, or an emergency, the automation should not send BOOK. That thread needs a human, and the public reply should not pretend a calendar is care. Keep a separate rule: no keyword match on HELP, CRISIS, or SUICIDE that fires a sales URL.

It is not a growth bot. Tools that ask for the Instagram password, then message strangers who liked a reel about anxiety, are how practices pick up restrictions and a reputation problem. Official API tools only talk to people who already contacted the account. Meta’s rules live in the Instagram messaging docs.

Therapist comments look similar on a Reel and are not the same URL.

  1. Booking. BOOK, INTAKE, NEW, CONSULT. The DM holds SimplePractice booking, Jane, Calendly, or a “new client” form. The EHR still holds the slot.
  2. Hours / location / telehealth. HOURS, ADDRESS, ZOOM, TELEHEALTH. The DM holds a page with times, the video link policy, and parking. Update this when you change hours.
  3. Insurance / fees. INSURANCE, FEES, SLIDING, COST. The DM holds a dated page of plans you take and a starting fee range. Do not let a bot invent coverage.
  4. Waitlist. WAITLIST, OPENING, NEXT. The DM holds the waitlist form. Turn BOOK off the week you are full.
  5. Resources that are not care. WORKSHEET, GUIDE, PDF. A public psychoeducation PDF is a file. It is not a diagnosis. Host it yourself. Do not mix it with INTAKE.

A Business or Creator account is required. Personal accounts cannot connect. If the practice is still on the clinician’s personal profile, that is the first ticket, and it is also a dual-relationship mess waiting to happen. Use a practice account.

Therapist comments are high intent and high risk. People type BOOK because they watched a two-minute Reel, not because they want a newsletter. If the DM takes twenty minutes, they have already booked the next listing on Psychology Today. If the DM asks them to describe their trauma in Instagram, you have a privacy problem, not a conversion problem.

Do not mix BOOK and INSURANCE on one keyword. A calendar and a coverage page are different jobs.

How we ranked these tools for therapists

Five criteria, same weight for every product:

  1. Comment-to-DM with a booking or intake URL - Keyword on a post or Reel produces a DM in seconds.
  2. Story replies - “Reply BOOK” on a same-week openings story has to work. Cancellations show up in stories, not in a 12-step bot.
  3. Official Instagram API - Instagram login, not a password form.
  4. Cost of a spike - What 200 BOOK comments on a viral education Reel costs. Flat DM buckets beat per-contact bills when a Reel about burnout hits.
  5. Time to first live automation - Minutes with a guided path vs an hour teaching an AI setter what “intake” means. A solo practice should not need a 12-step builder to send a calendar.

Last updated: 24 October 2026.

We sell DM Automation. Other tool prices are taken from those vendors’ public pages or a dated public quote of those pages. Respond.io is in the list because some group practices already live there. It is not ranked above ManyChat. SetSmart stays on the tracking list even though an AI setter is usually the wrong shape for clinical intake.

Respond.io: respond.io/pricing. SetSmart: setsmart.io/pricing. ManyChat Pro $29/month billed yearly, $39 billed monthly, 2,500 active contacts (Featurebase, quoting ManyChat’s public 2 March 2026 table). Source: ManyChat pricing.

LinkDM Free includes Instagram DM automation and story auto-reply. Comment auto-reply is a Pro feature (LinkDM pricing, October 2026). ReplyRush Free is 1,500 DMs/month without comment auto-reply (ReplyRush pricing, October 2026).

1. DM Automation - Fastest first BOOK or HOURS keyword

DM Automation is the product on this site. Free forever: 1 Instagram account, 100 automated DMs a month, comment-to-DM, story replies, keyword triggers, welcome message, iOS and Android. Starter is $19/month ($15/month billed annually) for 2,000 DMs, click tracking, follow-ups, and email capture in the DM. Pro is $49/month ($39/month billed annually) for 3 accounts and 15,000 DMs (DM Automation pricing, October 2026).

The practice switch is short if the live job is one keyword and one URL. Connect a Business or Creator account through Instagram login. Pick the education Reel. Set BOOK or HOURS, not both. Write the public comment reply (“sent you the booking link”) and the private DM with the SimplePractice or Jane URL. Publish. Comment from a second account to test. Screenshot the DM. Show the office manager.

Where it fits: solo or two-clinician practice, Instagram-first, you want a free plan that is not 25 contacts.

Where it does not: 100 DMs/month on free will not cover a viral burnout Reel. Upgrade before you post the noisy one, or pick a bigger published bucket (LinkDM Pro at 25,000 DMs). If returning clients already live on WhatsApp, ManyChat is number three. If a group already runs a shared inbox across five clinicians, Respond.io is the desk, not the first keyword.

The concession is honest. DM Automation is not SimplePractice. It does not do WhatsApp. It does not store a chart. That is why ManyChat stays number three.

Click tracking on Starter tells you whether BOOK taps actually opened the calendar. If they did not, the link is wrong, the EHR booking page is logged-out-only, or the caption promised a consult the calendar does not offer.

2. InstantDM - $9.99 Legend Pro when an education Reel spikes

InstantDM is number two on this site. Legend Pro is $9.99/month billed yearly with unlimited DMs (up to 800 an hour), Instagram plus Facebook, and story, Live, and next-post triggers. Trendsetter is $24.99/month billed yearly for the flow editor, Meta Ads, API, and webhooks. Trendsetter Pro is $49.99/month billed yearly. Free forever is 500 DM automations a month. Paid plans also offer a 7-day free trial, no card (instantdm.com/pricing, October 2026). Multi Starter is $160/year for 3 accounts.

Education Reels spike in a way office-hours posts do not. One Reel about panic attacks at 9pm can produce hundreds of BOOK comments before morning. A contact-billed tool treats those people as CRM seats. Unlimited DMs on a flat $9.99 yearly-billed plan means the spike does not change the bill. You still respect Meta’s throughput. You do not pay per inquirer forever.

It sends payment links in DMs. Stripe or PayPal still process a consult deposit if you use a payment link. InstantDM is not a checkout and not an EHR. Do not put a diagnosis form in that payment link.

Use InstantDM when you want unlimited DMs on a small flat bill. Use LinkDM when you prefer a published monthly DM cap. Use ManyChat when WhatsApp has to sit in the same canvas. Use DM Automation when you want this site’s guided form and a $19 Starter with click tracking.

A second InstantDM mention belongs here because “unlimited DMs” is not “unlimited openings.” The EHR still has to say no when the week is full. Change the DM to a waitlist URL the same afternoon.

3. ManyChat - Keep it if returning clients already WhatsApp

ManyChat is number three. Pro is $29/month billed yearly ($39 billed monthly) with 2,500 active contacts on the 2 March 2026 plan set (Featurebase, quoting ManyChat’s public table). Free is 25 active contacts per month. Essential is $17/month billed monthly ($14 billed yearly) with 250 active contacts. Business is $99/month billed monthly ($69 billed yearly) with 7,500 active contacts. Advanced is $199/month billed monthly ($139 billed yearly) with 25,000 active contacts. AI is included on Pro and above on the new model. Extra contacts are billed on top. See ManyChat pricing.

Keep ManyChat when Instagram is one channel among WhatsApp and the practice already texts appointment reminders there. Leave ManyChat when you only needed BOOK on a Reel and the contact meter is the thing you came here to escape.

The cost trap is contacts, not DMs. If a Reel produces 300 new people, those people can stay on the contact meter. Featurebase’s worked example of a 10,000-contact ecommerce brand on Business plus overages plus WhatsApp delivery lands at $307.50/month in their October 2026 article. That is an example, not your quote. A solo practice will not look like that. A group with a viral clinician Reel might.

Where it wins: WhatsApp plus Instagram, team routing, a flowchart you already built for waitlists.

Where it loses: a single BOOK keyword billed as 300 new contacts, and any flow that asks for symptoms inside chat.

4. LinkDM - $19 Pro if one waitlist Reel is loud

LinkDM Free is 1,000 DMs/month on 1 account. Pro is $19/month for 25,000 DMs on 3 accounts. Platinum+ is $99/month for 300,000 DMs on 10 accounts (LinkDM pricing, October 2026).

Free will cover a quiet week of HOURS. Free will not cover a waitlist announcement if comment auto-reply is the public half, because that checkbox sits on Pro. Story auto-reply is on Free.

Where it fits: you want the invoice to say DMs, and one Reel already proved the volume.

Where it does not: WhatsApp, a shared clinical desk, or an AI intake.

Do not buy Platinum+ because you have ten consult inquiries. Buy Pro after Free or Starter actually ran out.

5. ReplyRush - $10 Lite, cheapest paid start

ReplyRush Free is 1,500 DMs/month. Lite is $10/month for 7,500 DMs. Boost is $25/month for 40,000 DMs. Max is $100/month for 400,000 DMs (ReplyRush pricing, October 2026). Comment auto-reply is not on the free plan.

If the ManyChat quote was the thing that stopped the practice owner, $10 Lite is the cheapest paid Instagram bucket on this list. 7,500 DMs is more than DM Automation Starter’s 2,000, at a lower sticker. Starter still wins if you want click tracking and email capture inside the DM on this site’s product.

Where it fits: one practice account, paid comment-to-DM, a booking URL.

Where it does not: a five-clinician inbox. Boost is 3 accounts. Max is 5. Groups that already live in a desk should look at Respond.io, not Max.

6. Chatfuel - FAQ on parking and telehealth, not symptoms

Chatfuel Business is US$18/month billed yearly (US$216/year) with US$20 of AI credits per month. Agency S is US$90/month billed yearly for 3 companies (Chatfuel pricing, October 2026). Every plan starts with a free trial, no card.

Where it fits: parking, “do you do telehealth in my state,” “do you take new clients,” repeated in the same words.

Where it does not: symptom questions. Do not let an AI co-pilot invent a diagnosis or a treatment plan. Hand those to a human. Chatfuel can still send the intake URL and say a clinician will confirm fit.

See Chatfuel alternatives for Instagram DMs if you already opened that canvas and only needed a calendar.

7. Respond.io - Group-practice desk from $79

Respond.io Starter is $79/month ($948/year billed yearly). Growth is $159/month. Advanced is $279/month (Respond.io pricing, October 2026).

Where it fits: a group inbox that already covers WhatsApp, Instagram, and email across clinicians, with humans finishing scheduling changes.

Where it does not: a solo sending a Jane URL. $79 Starter is the wrong first practice tool.

8. SetSmart - Usually the wrong shape for clinical intake

SetSmart Light is $25/month for 250 AI messages. Pro is $97/month for 1,000 AI messages (SetSmart pricing, October 2026).

Where it fits: a non-clinical consult business that wants an AI thread to collect date and a form URL. That is not most therapy practices.

Where it does not: BOOK on an anxiety Reel. You would be paying AI messages to send a calendar, and you would be tempting the model to “qualify” symptoms.

SetSmart stays on this site’s tracking list. It is not an EHR and it is not ranked above ManyChat.

Keywords that should never share a flow

BOOK vs INSURANCE. One is a slot. One is a coverage page. Mixing them sends a Tuesday 3pm to someone who wanted to know if you take Aetna.

BOOK vs WAITLIST. When the week is full, swap the URL the same afternoon. A live calendar with zero slots trains people to ignore the next caption.

HOURS vs BOOK. Closed Fridays should not receive a booking link that still shows Friday.

INTAKE vs WORKSHEET. A public PDF is not an intake. Do not collect a history in the same keyword as a breathing-exercise file.

HELP / CRISIS vs BOOK. Never. A crisis comment is not a lead. Human, or a public line that points to emergency services. Do not automate a sales URL onto those words.

Write the public comment reply in the practice voice. “Booking link’s in your DMs” is enough. Do not write a paragraph in the public comment. Templates for shorter lines live at Instagram DM templates.

Privacy rules the tool cannot save you from

Instagram is not the chart. Do not ask for symptoms, meds, or a trauma timeline in the DM. Send the EHR intake.

Do not claim HIPAA because a vendor used the word “secure.” We did not verify any tool on this list as a HIPAA-covered therapy product for Instagram Direct. Treat Direct as a public-ish marketing channel that happens to be a private thread.

Practice account, not the clinician’s personal profile. Dual relationships start in DMs on personal accounts.

Deposits. If you take a consult deposit, Stripe or PayPal process it. The DM tool sends the payment link. Put the refund rule on the page, not in a 400-word bot.

Minors. If you do not see minors, say that on the booking page. Do not let BOOK imply you do.

State lines. Telehealth is licensed by state. The HOURS or TELEHEALTH URL should say where you can actually see people. A bot that says “yes we do Zoom” without the state list creates a board complaint.

Two tools on the same keyword during a switch. Two DMs. One tool, then cutover.

Password apps that message people who liked a Reel about depression. Official API only. See the Instagram messaging docs. If a warning followed a password app, read “We suspect automated behavior.”

What an education-Reel spike costs

A Reel at 300 BOOK comments, one DM each:

  • DM Automation Starter: 2,000 DMs/month at $19. 300 fits. A second viral Reel the same month may not.
  • InstantDM Legend Pro: $9.99/month billed yearly, unlimited DMs. The spike does not change the bill.
  • InstantDM free: 500 DM automations/month. 300 fits. 600 does not. Upgrade before you post the noisy Reel.
  • LinkDM Pro: 25,000 DMs at $19/month. 300 fits. Free’s 1,000 fits the count but comment auto-reply is Pro.
  • ReplyRush Lite: 7,500 DMs at $10/month. 300 fits.
  • ManyChat Pro: 2,500 active contacts at $29/month billed yearly. 300 new commenters is a chunk of the bucket if they were not already contacts.
  • Chatfuel Business: US$18/month yearly plus credits if the flow used AI turns.
  • Respond.io Starter: $79/month. Wrong meter for a solo.

Dentist Reels and restaurant Reels are different jobs. This page stays on BOOK, INTAKE, HOURS, and INSURANCE. Beauty chairs live at Instagram DM automation for beauty brands. Restaurants live at Instagram DM automation for restaurants.

Who is each tool for?

Solo, Instagram only, testing. DM Automation free (100 DMs) or InstantDM free (500 automations).

Solo, noisy education Reels. InstantDM Legend Pro at $9.99/month billed yearly, or LinkDM Pro at $19/month, or DM Automation Starter at $19/month.

Returning clients already on WhatsApp. ManyChat is number three.

Tiny paid budget. ReplyRush Lite $10/month for 7,500 DMs.

Parking and telehealth FAQ, not the chart. Chatfuel Business from US$18 yearly.

Group inbox. Respond.io Starter $79/month.

AI qualifier. Usually skip. SetSmart from $25 Light is the listed option if the business is a non-clinical consult.

How to choose in 10 minutes

  1. Start with DM Automation (own tool on this site). Free 100 DMs/month, Starter $19, Pro $49 for 3 accounts.
  2. If you want unlimited DMs on a flat $9.99 yearly-billed plan, InstantDM is number two.
  3. If returning clients also need WhatsApp in the same builder, ManyChat is number three.
  4. Lowest paid Instagram comment-to-DM: ReplyRush Lite $10.
  5. Published cap on 3 accounts: LinkDM Pro $19.
  6. FAQ bot: Chatfuel Business from US$18 yearly.
  7. Multi-channel desk: Respond.io from $79 Starter.
  8. AI consult thread: SetSmart from $25, usually the wrong first practice tool.

If two tools fit, pick the meter that matches last month’s noisest Reel. A waitlist week and a quiet hours post are not the same month.

Collecting emails for a newsletter, not an intake, lives at Instagram DM email list. That is a magnet PDF, not a chart.

FAQ

What is the best Instagram DM automation for therapists in 2026?

DM Automation first (this site: free 100 DMs/month, Starter $19/month, Pro $49/month for 3 accounts), InstantDM second (Legend Pro $9.99/month billed yearly, free forever 500 DM automations/month), ManyChat third if returning clients still need WhatsApp. Then LinkDM, ReplyRush, Chatfuel, Respond.io, and SetSmart. Ranked 24 October 2026.

You send the URL, not the appointment itself. The EHR still holds the slot. Test from a second account. If the page requires a login the client does not have, the tap is wasted.

Should I let a bot ask about symptoms?

No. Send the intake URL. Instagram Direct is not the chart. Do not collect a history in a marketing thread.

Does InstantDM replace SimplePractice?

No. InstantDM is Instagram DM automation at $9.99/month billed yearly for Legend Pro. It sends booking URLs, hours pages, and payment links. SimplePractice still holds the appointment if that is your EHR. Stripe or PayPal still process deposits.

How much is ManyChat Pro in 2026?

$29/month billed yearly, $39 billed monthly, 2,500 active contacts, on the 2 March 2026 plan set (Featurebase, quoting ManyChat’s public table). Free is 25 active contacts. See ManyChat pricing.

What do I do with crisis comments?

Do not fire BOOK. Hand the thread to a human. Do not automate HELP, CRISIS, or SUICIDE onto a calendar. Instagram is not emergency care.

Can I automate DMs from a personal Instagram account?

No. Switch the practice to Business or Creator first. Keep clinical work off the personal profile.

Is it safe to auto-DM people who comment on therapy Reels?

On official API tools, you are messaging people who already commented or replied. That is the allowed pattern in Meta’s Instagram messaging docs. Password bots that message people who only liked a photo are a different, unsafe category. Free-plan limits: free Instagram automation tool 2026.

Do I need WhatsApp to take new clients?

Only if your clients already talk there. Instagram-only comment-to-DM books consults without WhatsApp. ManyChat is third when WhatsApp is a real channel.

What if the week is already full?

Turn BOOK off or change the DM to a waitlist URL the same afternoon. A live calendar with zero slots trains people to ignore the next caption.

How is this different from the coaches page?

That page is calls and programs. This page is private practice: BOOK, INTAKE, HOURS, INSURANCE, waitlist, and the hard no on symptom bots. Same tools, clinical URLs.

Next step

Pick a free plan, connect one Business or Creator practice account, and ship one comment-to-DM on one live education Reel. BOOK or HOURS, not both on day one. Comment from a second account. Screenshot the DM. Then pay for a bigger bucket only after a real booking tap hits the thread.

Your next Reel can sell while you sleep

Connect the account, pick a keyword, write one message. That is the entire job, and the free plan covers it.
